Student Transfer Information and Forms

  • The State Department of Education requires that the following form be completed to apply for a transfer: Student Transfer Application.

     

    Parents needing technology access to print or complete the form can visit the Haworth Public School District. Paper copies of the form can also be picked up at the Haworth Public School District located at 300 N. Maple, Haworth, OK.

     

    • Decisions about transfers for the 2024-2025 school year will be made starting July 1, 2024. We will contact parents directly about the approval status of their child’s transfer.
    • Mid-year transfer requests will be considered within [5] school days of the district receiving the application.
    • New transfers are accepted on a first-come, first-served basis. Students who attended the district on a transfer during the 2023-24 school year will be automatically accepted pending approval from the district. The law also gives preference to children of active-duty military personnel.
    • To accept a transfer, enrollment in the grade the parent requests must be under the district-established capacity. The district’s capacity and vacancy numbers for each grade and school can be found at this link. Copy of Haworth Public Schools Student Transfers
    • Transfers may be denied based on capacity, attendance, and discipline issues.
    • Transfers for siblings must be considered separately.
    • Transfer students must abide by eligibility requirements established by the Oklahoma Secondary Schools Activities Association (OSSAA) for OSSAA-sanctioned activities.

     

    • When capacity allows, and after a transfer application is approved, the guardian must complete our district's enrollment registration.
    • Transfer decisions may be appealed. Refer to Board Policy for the appeal process.

    Using the Official DHS Website (Recommended):

    1. Go to the Contact Us page: Visit oklahoma.gov/okdhs/contact-us.html.
    2. Use the Interactive Map: Click the map to zoom in or out, or select your county to see location details.
    3. View the List: Click the arrow in the black banner on the map to see a list of all office locations. 

    Other Ways to Find Offices:

    • Search by County: The DHS site also allows you to find offices by county for specific services like Child Support.
    • Mapping Apps: Search on Google Maps, MapQuest, or Yelp for "Oklahoma Human Services" or "DHS" in your city (e.g., Oklahoma City, Tulsa). 

    Important Notes:

    • DHS is now officially called Oklahoma Human Services (OKDHS), but many people still call it DHS.

    If your ride is 15 minutes late, call “Where’s My Ride?” at 1-800-435-1034 or 711 (TDD).

    Lodging and/or Meal Services: If you must travel a long distance that requires an overnight stay in order to access Soonercare approved medical services with a specialist doctor, then you can contact the Population Care Management Division at 1-877-252-6002 to request help with lodging and/or meals during your trip.

    *LIFT CAA Transit: Demand/response routes operate in Antlers, Hugo, Idabel, Clayton and Broken Bow. Local demands/response routes run from 8:00 a.m. to 4:00 p.m.. Monday through Friday. Demand/response services are within city limit and curb to curb.
